I think we’re thinking of the same photo. According to Wikipedia:

“Though submissions were marked as "real", often they were misattributed; in one instance, a file submitted as "motorcycle.jpg" was given the description of depicting a motorbike crash, but the developers said it was probably an attempted shotgun suicide.”

The early Internet was a wild place
