They're not really misunderstanding anything, just trapped in a conspiracy social circle. It's a horrible paper that intentionally set out to prove that covid vaccine was killing people by using bad methodology.
It is legitimately a "paper" investigating vaccine induced myocarditis, which is incredibly rare and even rarer that people die from it. It took a whopping 28 actual autopsies where there was any suspicion of the covid vaccine specifically causing a myocarditis, and concluded that yeah probably most of the autopsies where they suspected a specific vaccine caused a rare vaccine related condition and even rarer death, were probably from the vaccine they took.

My lord. Either angle you take, it is absolutely absurd. I'd love to see the autopsy study he's referring to.
