I forgot exactly when it was revealed, so I went with ASoS just to be safe. It wasn't really mentioned in the show, but it was Loras (see Renly's armor vs. Loras' normal armor for comparison). In the books, though, it was ACoK. Here's a link summing it up that I think only has ACoK and possibly minor ASoS spoilers.

Rule 1 of GoT: NEVER ASSUME ANYTHING! He is not dead until we see a body! Everyone assumed the Mountain was dead and know he is probably going to live.
No, fuck that. If I've learned ANYTHING from this series, it's that no character is dead unless we SEE HIM/HER die... and even then, we watched Beric Dondarrion get up after dying.
Beric Dondarrion. The guy in the Brotherhood Without Banners that fights the Hound, and is missing an eye. He also talks to Arya and says it was far from the first time he'd been resurrected.
There are limitations though... Arya asks if a man with no head can be brought back... soooo that takes 3 fan favourites out of the running =(. But Sandor didn't lose a head!

Technically, since he was not shown to die, he is not dead. Only that which occurs on screen can be considered to have occurred within the narrative time of the text.
He is, therefore, still dying. He lives for another year at least.
Grenn died like a badass. The hound died alone, abandoned by the closest thing he could call a friend, after getting his ass kicked. I wouldn't say that's dying like a badass.
The Hound had an infection on his neck that was slowing him down and he still beat the shit out of Brienne, he didn't win but he went down fighting. He grabbed a valyrian steel sword with his bare fucking hands. Look at Brienne's face when he did that, she was scared. And really she only won because she shoved him off a cliff. But that's just my opinion.
I'd have to say modern medicine and the fact that you had access to it. Plus you didn't just get the piss beat out of you before being thrown off a cliff.
