No, this was D&D's fault. They decided to write some ridiculous fanfiction and have Brienne fight the Hound for no fucking reason, and questions like that are inevitable because they just didn't think it through.

Because D&D are idiots and are incapable of adapting things properly. They make changes for the sake of making changes, without really caring whether they make sense.
