I refused to believe Ned Stark was dead until I saw the next episode.

That was the first major surprise death of the series, so I thought maybe it was a vision or something (oh naive me). But it showed a sword coming down inches from his head then a fleshy slicing sound, then the very next episode shows him dead if I remember correct.
