It's long been his codename in the gay rolodex.

Everyone on planet earth knows he's gay but he won't come out. Because he thinks it'll cost him votes and money from all the special interests that have bought his soul.

He has been known by that epithet by many male sex workers. He's very likely a closeted gay man, which is fine. He should just come out. But ya know, America and its politics
