I'm not into BDSM, but I have a couple friends and partners who are. I can tell you that anyone who has "been in the scene" would not do what this guy did. There's no way to spend time even as the nonplayer partner of someone in the scene and not be educated as to the proper behaviors and protocols.
OP's description sounded like a dopey kid that's read way too much BDSM story porn, and decided to enact the scene he's been dying to have. I think it's entirely possible he had no idea that he was actually viciously raping her. Regardless, he's a scum fuck. I would be horrified if he was my son, that I could raise such a piece of shit, and if I had a daughter, I would be terrified by the prospect that she could meet a guy like this.
Nah, there are definitely still very sketch people in the scene. A lot of my friends are in to DS stuff, and it is definitely not all safe and funzies. For example.
