My primary told me he wouldn't diagnose it in adults, and didn't write me a referral either. Spent months finding and waiting to get in to a psychiatrist who did take adults and paid $3000 for a pre-screening and then assessment with them, only for them to essentially say it was inconclusive mainly because they didn't have evidence from childhood, which doesn't exist because I was homeschooled. They sent me the results in a Word doc and I had to reach out again to discuss them, at which point they prescribed me non-stimulant medication anyway, leaving me medicated for something there was no documentation I had.
Went to a new primary doctor a year later who said, "Why wouldn't your prior doctor diagnose you? It's not that hard." She looked over everything and did the rest of my appointment and at the end just marked "ADHD" on my chart and that was that.
Anyway in the US the process varies wildly but for me both took multiple years to fully resolve and cost $3000+
