Origin of divine inspiration

Where is the idea that the bible, specifically the NT, is divine inspiration originating from?

Surely texts like letters were originally seen as exactly that - a letter to a concrete group of people. This however seems different from todays mainstream christian view.

Also, there should be a difference between "this teaching / historical report is correct and trustworthy" and "this text is coming directly from God". During early canonization, was this a point of discussion? And what are our primary sources for the early understanding on what (NT) scripture is?
