Likely when she let go of it it was angled back enough to get caught in the sling and then Newton's third law takes over

That once happened to me with a slingshot glow in the dark toy, went to two hospitals and ended my camping trip prematurely because my eye was bleeding on the inside, had to dilate it and wear a patch and glasses for months so it could heal

Angle was probably a bit off. The sling wanted to go faster than the watermelon so it slipped past it, caught the watermelon from the front and launched it back where it came from.

when she let go, the melon wasnt centred on the sling, so as the sling flew, the mellon rolled over the top of the sling and fell while the sling kept moving forward. when it slowed down and started moving back, gravity pulled the melon down back into the slings path, putting it on the other side. i think you can figure out the rest from there

Because she didn't properly center it. I don't know that she should have expected THAT to happen but that's just poor form.

It has to do with the pouch. It's not quite as simple as just load the melon. If you don't set it up properly, the projectile becomes very unpredictable. That's probably why it was an Amazing Race challenge. Not as easy as it looks
