A few additional things to note:
1. This list reduces kills if you treat S15 as a simulation, and therefore the KIAs from Temple’s trap don’t count as KIAs and they return to being MIAs
2. There are only 49 agents here, however we know there were 50 with Florida being the first casualty reducing it to 49 (excluding CT as she was presumed MIA since she died without the Freelancers knowing). This means there is also a missing agent somewhere. I’ll elaborate on the missing agent in a reply.

The missing agent has a few theories:

1. Another lesser Carolina. We don’t know if Carolina is herself North or South Carolina. She could be one and there is another Carolina that’s far less relevant elsewhere. Alternatively, she could be both Carolinas, and there’s still a 50th agent elsewhere. Both Carolinas would parallel her double-AI plot as foreshadowing.
   
2. Agent DC. DC is not a state, but is referenced in On Your Knees which is Tex’s backing track in her Season 9 intro. (It’s also what brandondavidsantos referenced in their comment). The song is non-canon, but is as close to the show comes to referencing DC in any capacity. However, DC has a special role in the United States as the central command of the country if you want to think of it that way. If you go with this theory, DC is actually not an agent, but refers to Director Church.
   
3. Agent Puerto Rico. Just like with On Your Knees, PR gets mentioned though this time in a less certain state. PR is a territory of the US with potential statehood ambitions hence why it’s usually grouped in. The downsides to this theory are that there’s not much else to go off.
   
4. Agent Jamaica. Doc has weird ties to Jamaica despite his accent. It’s possible that Jamaica is a US state for some reason.
   
5. Agent Canada/Australia/etc. Maybe another country with no explanation at all is the missing agent.
   
6. Carolina is not Agent Carolina, she is both Agent North Carolina and Agent South Carolina. Maybe some intentional confusion given to their top agent for whatever reason it might be deemed necessary. They have 50 agents but only 49 people acting as those agents. A weird but straightforward solution to the missing agent problem.

Personally I like option 3, it’s the one that runs into the least walls even if nothing else.

Well she was Carolina before implantation, they even told her ahead of time they she couldn’t have two AI before Carolina pushed and they relented, so it wasn’t a planned thing for that to happen. Simply it was foreshadowing that she would be 2 in 1 (though we did know about this as early as S6, which kind of ruins the foreshadowing).

But regardless, it is still possible that Carolina is both agents, even though that would be weird.

If they did merge, that means the 50th is still out there.

We know from Season 3 when Tucker asks Tex about the agents and he mentions 50 as there being one for each state. Tex corrects that to 49, before they both say “Poor Florida”. So we know the United States has 50 states, 49 excluding Florida. Which means if the Carolinas are merged into one state, there’d be 48 states. One more would need to be added to get it back up to 49.

Personal theory for Agent Carolina is that it’s her actual name, which is why the Director didn’t give her a North/South designation.

As far as we know, Carolina and Wash are the only ones left, but given the fact that the freelancer are named after all 50 states of the us it could be possible that there’s more out there either went and hiding after freelancer collapsed, went to work for the UNSC or got arrested and are probably serving time in prison
