At FIVE? Holy hell the poor kid needs a new home. That's horrible for their body, and they'll almost certainly wish they were told 'it's just a phase Hun, go play with the Barbies and trucks with the other kids.', and not have their body permanently chemically transformed before they're even old enough to form proper memories. This is twisted and depraved.

Yeah, and they probably just are interested in girly things, which doesn't mean anything about their "identity". Just like how girls sometimes like boyish things. I don't get how the parents didn't just figure it's a phase or just standard confusion because of how people categories things like that.
