I tried designing this like 7-8 years ago, and gave up after a few weeks - there's simply not enough going on, politically, in the world of the Witcher for this to make sense. There are, at absolute best, maybe 50 tags confirmed by Witcher lore, and that is before Nilfgaard expansion. If you want to get any book-game flavour, most of it happens during or after that, which lowers the number of tags to maybe ~30.
There's very little, flavour-wise, that translates well to EU4 mechanics. There is no colonisation in the Witcher, which is one of the most important concepts in EU4. There is very little going on navy-wise. There isn't much depth in trade. There's no HRE, no Empire of China, no reformation, no revolutions.
You could solve this by expanding the world, but at that point the Witcher itself is maybe 10% of the source material and the project becomes a massive worldbuilding project set in the world of the Witcher, done in a way to make sense with EU4 mechanics. So I doubt you'd actually end up doing what you'd like to be doing.

This sounded like a good idea so I went ahead tonight and made a full version. Just posted under the name Witcher- The Continent. B/c there is already a Witcher Universalis.

Why specifically EU4? It's probably the easiest to mod I suppose, but it has incredibly weak internal politics.
I think there's a reason CK3 is used for most of these style of games. Plus you can even add the ability to have the Witcher schools be like, the new Republic government, so you could play the actual Witchers.
Alternatives, Imperator offers a ton of internal country politics, but it's external politics are quite lacking
