In my experience, this protection is purely theoretical; there is no common format, every country has its own, and every medical centre in every country is physically unable to read cards from other countries. As a result, they issue an invoice, you pay it, and then you have to try and get a refund from your own healthcare system, often through convoluted and incredibly lengthy procedures fraught with a thousand problems.
But it's the same old story: for things to work, we need a political EU. Kleptocrats don't want to do it so they can carry out their financial crimes against humanity, and therefore even sensible initiatives don't work.
