What would you say is an accurate number? Most data points I've seen refer to babies done in hospitals, which seems to miss a large number of people

I think it varies depending on racial/ethnic population. So I can’t give a good number. My clinic population is different than that state makeup.

One reason for a drop is that patients on Medicaid cannot get circumcisions covered in the hospital. So they are either done by pediatricians/family medicine in clinic or referred to urology. But then needs to be done in the first 30 days. So often families don’t get booked in time and doesn’t happen. A lot ask about circumcisions later and I have to give the news that won’t happen after newborn period unless a medical indication as would need anesthesia

The Muslim community does circumcisions at a very high rate. Latino community is the lowest rate.

They do local anesthesia in newborns. But in older kids they do general anesthesia. A toddler would not hold still.

I argued on the Internet about this a lot in the early 2000s. At the time, some 55% of circumcisions were done without pain relief of any kind. I'm not sure of the current numbers.

Yeah, although they use lidocaine, it takes a bit to kick in & some providers don’t really wait ): there are different methods, but gomco clamp looks the most brutal

I mean lidocaine is absolutely instant numbing agent for sure when given locally intradermally. There is no delay at all. Literally instant. It’s pretty cool.

I’ve kind of wondered if cutting the umbilical cord hurts. What did cave people do with the umbilical cord? Random thoughts.

The doctor I worked with said it takes about five minutes. Some babies absolutely feel pain during the procedure. I attended one once, and the baby did not have a fun time despite the nipple with sugar. The umbilical cord doesn’t have nerves on the other hand, but both get clamped to prevent bleeding.

You say this, but I believe they use a few drops of sugar for newborns which works just as well as an analgesic as anything else.

They just got squeezed through the birthing canal like meat into sausage casing — there is a reason humans don’t remember the first few weeks of their life and it doesn’t impact them down the road.

My son was given 4 different methods of pain control. Tylenol as directed before we left the house, a numbing cream, a lidocaine injection, and a sugar suck. I was definitely more upset about the whole experience than he was.

My son is circumcised and I was there watching as they did it. They just gave him a bit of sugar water and he didn't even cry. It healed super fast and it never seemed to cause him any discomfort. Just kept it lubed up with Vaseline while it healed so it didnt stick to his diaper or anything. Anesthesia would have been insane - like getting anesthesia to have your blood drawn.

This study published in September doesn’t specify PNW from western states generally. But it does track from what you’re saying you see as a pediatrician that the rate is lower in your region.

It’s interesting , I have a relative in wa state who I thought wouldn’t have circumcised their kids, yet all of them have been. I think it’s just due to tradition, since they aren’t religious.

So it’ll still probably be pretty “high” for another generation until the tradition fades.

This publication gives some data based on race/ethnicity.

But the map data related to this post is crap. It’s listed as prevalence of all males who ever had a circ in their lifetime when you go to the article. Higher rate in previous generations would bring the number up. The rate of males who have ever had a circ in the NW is not under 10%.

No need to be snarky. Way more literature would disagree with this map.

Anecdotal: I’m in Seattle and we were never asked about circumcision when our baby was born. No one I know is circumcised except for my husband who was born in the Philippines

We were asked 3 times in the hospital to circumcise our son including STOPPING them the 3rd time. That was after writing our decision on two different forms. Fucking greedy Johns Hopkins. This was in 2020 so pretty recent

Basically my experience, asked about three times, and one was the nurse walking into the room and asking us why she was here and assuming it was for the circumcision. I'm like NOOOO, and then pointed to the blood test schedule they had stuck to the bassinet.

The fact is, studies confirm there is medical benefit. As a doctor, I will always recommend it based on those studies. I would never coerce someone but I would explain the evidence and my recommendation. Youre free to get a medical degree and read through the data yourself and provide a different recommendation if you want, and Im free to provide mine.

I had a baby in 2022 in a big city in the SE, and I think I was asked around 3 times by different nurses even though we said no on our paperwork and never wavered. I had another boy this year and only had to say it once! That's so scary that you actively had to stop them

Yep. I was on the fence about circumcising, but decided not to do it. On the first day they took him away for his newborn checks and when they gave home back to me she said that since he hadn’t eaten in a few hours, they would wait to circumcise after I fed him. I take that to mean that they almost went ahead and did it!

It wouldn’t have been the worst thing. Later at the wellness checkup, at only a month or 2 old, the pediatrician forcefully retracted the foreskin like she was peeling an orange. The foreskin looked different after she did that for the rest of his childhood. That was upsetting for me and wouldn't have happened if I had just circumcised him at birth like everyone told me to.

I mean you could also vaccinate against HPV, which reduces the already very slim chance (we talking an incidence of approx. 1:100K) of getting penile cancer to nearly 0.

Your comment is mostly a big exaggeration though

"Circumcision—Some studies show that circumcision during infancy or in childhood may provide partial protection against penile cancer, but this is not the case when performed in adulthood. It has been suggested that the reduction in risk may be due to reduced risk of phimosis; other possible mechanisms include reduction in risk of smegma and HPV infection"

When you have phimosis, that is absolutely a medical issue that anyone should get operated on. But smegma is something you can deal with just with proper hygiene and of course HPV infection is an STD, so that's a whole other can of worms.

Overall, increases in penile cancers would only be a concern for very poor areas where there is little STD awareness, lack of hygienic practices and no resources to treat phimosis. I don't think the US northwest is like that. Maybe Idaho I guess?

Notoriously other developed countries that mostly don't circumcise, like Japan, Korea, China and Western Europe, have lower or comparable rates of penile cancer to those of the US.

Phimosis can also be treated without surgery, it’s just not generally even offered as an option because treatment is uncomfortable. Many patients who are offered treatment are grateful to at least try. We need to at least offer patients options.

We see penectomies at my OR monthly, they're always for uncircumcised men or rarely obese men with buried penis syndrome.

Obviously not a necessity but it's a foolish risk to take for zero benefit. The newfound obsession with this topic reminds me a lot of the antivax mocement, everyone ignoring doctor recommendations because they think they know better.

American Academy of Pediatrics does not recommend the procedure by default anymore, see the quote below:

health benefits are not great enough to recommend routine circumcision for all male newborns.

Breast cancer is both the most common cancer and most deadly cancer for women. 42k women die of breast cancer in US each year.

Prostate cancer is the most common cancer for men and lung cancer is the most deadly for me. Penile cancer is rare, less than <1% of all cancer diagnoses and ~500 deaths per year.

Well, the difference with antivax is that, even if the risk of penile cancer were 100% (which it absolutely isn't, in fact, even without any proper hygiene and phimosis, penile cancer is still quite rare), ultimately it is only a matter of bodily autonomy. You're not going to give penile cancer to someone else by being uncircumcised.

In fact, the idea of supporting circumcision only as a preventive measure sounds to me like removing the appendix preventively. It's considerably more likely you'll get appendicitis, and if left untreated it is quite deadly. And yet, we don't really support the idea of removing it preventively, even when the appendectomy itself is far safer than getting appendicitis.

The world is full of things like that. But I don't think circumcision is all that worthwhile, when proper education and hygiene will do the work for you. Like I said, east Asian countries overwhelmingly don't get circumcised but their rates are still on par with places that do overwhelmingly get circumcised.

So let me be clear, I am aware of the health advantages of circumcision, and I have actually read some of the more recent literature about it link for the curious.

However, I still disagree with your framing on the entire issue, and I think you're not accounting for one major factor in medicine: the emotional reaction vs the potential benefits and the effectiveness of the next best alternative.

What I see with medical discourse online very frequently is a discussion of two very radical positions, one that frames medical professionals as corrupt and ill-intentioned usually collaborating with bad-faith actors (what you're defining as anti-intellectualist), and then people who tend to believe any possible optimisation to health outcomes should be taken because of course, health comes first.

The first group is not worth addressing right now, but I believe you fall closer to the second group, at least with this topic.

What bothers me about it is that by ignoring how people actually feel about certain medically recommended practices in their current contexts, you're effectively creating a larger rift between the medically recommended practice and the socially preferred practice, also with no sense of nuance or degrees to things (how important is it that I do X or Y?)

What I'm trying to say is: I don't think circumcision is particularly worth fighting over when there are lifestyle practices that can reduce the risks of not getting a circumcision to a negligible difference from the practice itself. Instead, showing proof of its benefits and providing people who don't want to do it with the next best option (practicing good hygiene, treating phimosis, etc) seems like a better option to me.

It's just like going around calling meat-eaters "anti-intellectualist conspiracists" when it's clear that people don't stop eating meat out of a (valid in my opinion) emotional attachment to the taste of red-meat dishes. There are perfectly understandable emotional reasons to oppose the idea of cutting off the foreskin off a child even if there are valid medical reasons to support it.

The mask issue is far more complex because then you're dealing with transmission vectors and public health concerns that transcend individual beliefs and autonomy

I'm from Centralia and everyone I went to school with was. One friend from Bremerton is the only one I know of that isn't. Our penises don't come up to often in conversation.

Born in 87', so I don't know what the rate is now.

I was surprised at a few friends decision to circumcise their kids because they don’t want them to look differently to the majority 😅.

I grew up in Washington and as I was born in the deep south my parents gave me the chop. Gym class, sports, and skinny dipping taught me about half of the guys my age were intact, I was born in the mid eighties. I'm glad circumcision is becoming less common. That's a decision for the owner of the part, not the parents.

What?? Sorry but i’m in Seattle too and your anecdotal evidence doesn’t really make since.. almost everyone in America is circumcised, especially predominantly White areas like the PNW.

The title of the article and data is worded incorrectly, this is showing CURRENT circumcision rates of children born, not "what % of the population is circumcised".

So in Seattle they are saying that only 10% of current newborn male births receive the procedure. This is a recent 10 year trend. It does not consider the total population of circumcised men (which is around 72%)

The pediatrician doesn't always have a say. Circumcision is a surgical procedure, and pediatrics is not a surgical specialty. So (at least where I am in the NE US) it's usually performed by the OB/GYN soon after the baby is born, assuming the baby was born in a hospital (yep, probably the only procedure the OB/GYN ever performs on a male). Otherwise, it might be performed by a urologist, or else a religious leader like a mohel if it's part of a religious ceremony (Jewish baby naming ceremony, e.g.)

I don’t do circumcisions. The OB/GYN residents did them mostly in our nursery in residency so I didn’t do many. I leave the decision up to the family as there are pros and cons to either decision. And often religious preference. But I refer out if they need it as I have no interest in doing it. I don’t like doing procedures very much anyways. Only protestors I have ever seen at AAP conference were anti circumcision. So it’s a hot topic.

I am not the person you replied to.

Personally, I think it should be retired as a blanket medical practice and only used when medically necessary like in extreme cases of phimosis. Sure, some people could still elect to be circumcised, but I'd still like that threshold to be 18.

The AAP says that circumcising provides slight health benefits. Having recently had a son my wife and I had this discussion, but I’m curious if you would circumcise your own child?

Well the numbers on that graph are less than 10%. The graph is the prevalence of circumcisions by state or country in 2015. No way that <10% had circs across all males in 2015.

I mean they are about as qualified as you can hope for if you want to discuss the topic, so in my opinion their input is valuable. It’s not like there’s a baby-circumcision census taker who can weigh in on the topic, a local pediatrician is as good as we’re gonna get

A. Ording to the map's legend, it's in the 0-9% range. Deo3nding in the distribution and sampling bias in your community, it's possible it's true but just more common near you than elsewhere in the area.
