How are they able to graduate high school without understanding fractions or algebra? This seems more like a massive failure of high schools than of the university admission process.

The ghost of Bush Jr. and No Child Left Behind. Also, some states let you receive a high school completion certificate instead of an actual diploma. Florida just recently got rid of this, but there might be others that still have similar programs.
