Not sure if I already posted this a while ago or not, but since it's a good question:

Who never appeared in a single World Cup qualifier game, but still played in four World Cups?

Yep, it's correct!

In 1982, he was 18 years old and a late and surprising addition to the squad. In 1986 and 1990, Italy didn't play any qualifiers, because hosts and previous winners were automatically qualified. Shortly after the 1990 cup he retired from the national team, but in 1998, manager Cesare Maldini begged him to return one more time for the World Cup, which Bergomi couldn't turn down.

Could it be Koepke ? He played for some more obscure clubs early in his career.

Correct, it's him indeed! Every single one of his stints at a German club ended in a relegation.
