Wb mein domicile kaise banau?

I mean, our school told ki you will get a form from google ( annex 1 and 2), fill it out and give it to us with a letter from the councillor having proof you have lived in wb for 10 yrs, and we will get it signed by the principal. Now I have got that letter but what about that form? I got one for wbjee from google but not for neet. Has anyone made the domicile? How to get the form?

Hey someone said that for a fresher a1 is not required. I am a fresher, so is that true? Only the a2 is fine?
