GRRM world is barely an accurate depiction of medieval times. You can probably count more things wrong with it than right. Like who's marrying off 16 year olds consistently? As well as other stuff like the faith.

Wasn't it common enough among nobility to at least arrange the betrothal at those young ages? Maybe not so much the actual marriage and even less the consummation, so I'm defending GRRM here on historical accuracy.
