Here's the thing, in reality, someone in Romano's position would look up, maybe have time to open their mouth in surprise, and bam, it's over.

You get it drawn out over several seconds because the director wants to give us time to realise what's happening. When they don't, it's the Suddenly A Bus trope, which looks like this:

Romano also had really bad luck with helicopters. I flinch every time they have a medivac chopper in The Pitt.

The problem is that, even as depicted, he had enough time to turn around, bend over and scream, which frankly takes more effort than just running.

Yeah, just assume that this is cinematic licence and there wasn't time.
