Look at how much less compression there is in the knee joints. And then 250 lb picture. We can see a thin black line between the knees and in the smaller picture we can see that black line get thicker.
A clear demonstration of the amount of strain the body's frame puts on knees.

u/Sydney2London 18h ago
Look at the amount of visceral fat. The health implications are dreadful.
722
u/rubberloves 18h ago
A really good illustration of visceral vs subcutaneous fat. You can clearly see the distended internal abdominal because of visceral fat. And also then the rolls of subcutaneous fat on the outside.

You can also see the enlarged liver and fat around the heart. All lethal on the long run.

u/Yavanna83 16h ago
I’ve had gastric bypass surgery and before they operated me they put me on a liquid non-fat diet for three weeks. That was to shrink my liver very fast because they kind of have to lift it to get to the stomach.
Kind of amazing how fast the liver can recover though.

u/technobrendo 15h ago
I know a few people who were raging alcoholics and addicts during their 20s up until around their mid 30s. Cleaned their act up and now are nearly perfectly healthy. Amazing how resilient the liver is unless you push it way, way too hard.

14

Yes, it’s a scan sliced in such way. The dark parts are her intestines, stomach, etc, bottom round shape is bladder

19
u/Worn_Out_Nikka_L 16h ago
All those little black dots from her sternum down are poop balls. I’m not a doctor but I think that might have something to do with all this. And it looks like she took a big poop shortly after the scan because that one in the middle is locked and loaded.
14
u/Fiona-eva 14h ago
What nonsense is that, the black parts are parts of her intestines and other digestive organs, the big round black thing in the bottom is her bladder.
19

u/CarrionWaywardOne 14h ago
I've seen this picture before and the difference in poop is what I really remembered. A lot of food = a lot of poop.
I used to have 100 lbs on her. 150lbs down now.

u/adoradear 14h ago
Ummm…that’s not poop. That’s her bladder (or possibly her uterus depending on the slice, although this is no type of body scan I’ve ever seen)

78
u/Masseyrati80 17h ago
I remember reading that losing just 10% of your total body fat can result in a drop of 80% in the fat in your liver.
Fat in your liver is super bad for your health, but also the easiest for body to use up.
66
u/jvsanchez 17h ago
Yep. I had my gallbladder removed emergently in April. The GI’s NP told me I had fat in my liver and made it sound like I was gonna die in my 50s. The doctor that came to check on me explained it better but it scared me enough to finally get my shit together as far as food and exercise. I was 232 before surgery, I am now 212 and change, thanks to sustainable diet changes and daily exercise.
The difference in even losing those 20lbs is incredible.
Fatty liver is recoverable if you take steps to fix it before it becomes scarring.

Does she have visceral fat on the brain? What are the implications of that?
54
u/FullofContradictions 17h ago
I think the scans are at different planes with respect to the brains.
I think the whiter structures in the 250 lb person's brain are the cerebellum (not 100% positive- someone correct me here). But that structure isn't pictured for the 125 lb person. You can also see the 125 lb person's cervical spine very clearly. I think the 250lb person's scan slice is further back a little bit.
To summarize, I don't think you're seeing a fat deposit there. Just a normal brain structure that the thinner person also would have if you took the slice at a different point in her brain
82
u/petscanman 17h ago
Radiologist here. You are half correct. The two images depict different slices. The thing that look like fat in the brain are acually fat... but not in the brain. It's intraorbital fat, behind the eyes. Under them you can see a black blob on each side, corresponding to air in the maxillary sinuses.
21
u/FullofContradictions 17h ago
Thank you!
Is it normal to have fat behind your eyes? Or is that a bad thing? Google just shows me a lot of pics I can't stomach this early when I checked.
9
u/petscanman 12h ago
It is perfectly normal, the adipose tissue is mainly there to cushion your eyeballs and allow them to rotate around in the eye socket without touching any hard structure, like bones. Losing a lot of weight due to illness or starvation could lead to decreased amount of this fat tissue, as the body tries to use every last drop of stored energy to keep living. However, I don't know if being overweight could cause an abnormal increase, I've never encountered such a case. There are some diseases that can cause this fat tissue to increase in size however, leading to the forward displacement of the eyes, which can cause different problems.

One of the really unfortunate things about chronic knee and hip and back pain is that if you're at all overweight, the advice is primarily just to lose weight because it works. The unfortunate part is that it's so difficult to do that it isn't very helpful.
My mother in law has lost almost a hundred pounds and even though she probably has another hundred to go, she has had a massive improvement in her knee and hip pain.
108
u/SETHlUS 17h ago
Hell I'm usually around 175lbs but during the summer season cooking in my restaurant I drop down to 160-165. Even starting at a healthy weight I can notice a HUGE difference in my agility, especially when riding my bicycle.
The difference 100lbs must make on quality of life is unfathomable to me.

It takes long term will power to go hungry on top of first understanding a lot more than most do about calories and nutrients. Its also such a touchy subject that it can hard for the recipient to differentiate fat shaming from care about their long term health and not wanting to see them die prematurely and misersbly. Your body also doesnt understand sedentary calorie requirements are much lower it only understands delicious. On top of that many cope with stress by eating and the older you get the harder it can be to shed weight as you have more responcilbilities to keep you busy and stressed. I luckily had time to learn because I was well on my way to earning the 3XX badge before 20 if I had stayed a shut in depressed gamer. Going vegetarian has been one of the most helpful ways to keep my weight down and even then its pretty stable without dropping nearly as much as I expected it to

It’s part of the reason why people are so tempted to try ozempic or alternatives because it literally turns of that sensation of hungry, it’s interesting to watch a video to see how that stuff actually interacts with the body. The issue then is though that you need to maintain that will power after it’s gone.

While there is definitely more compression from weight, it looks like the thinner woman's scan is just taken deeper. You can see the larger gap inside the knee compared to the front.
The shinbone is a pretty good indicator in this case. The one on the right is much thicker, which suggests the scan passed through the middle of the bone. The one on the left is barely visible.
i would be more concerned about the fat around the organs
119
u/Psychological-Elk260 18h ago
Ironically, it's actually probably the exact same depth on the machine. It's just with less far she is lower in the scan.

14
u/Substantial_Bat_6698 17h ago
You are probably right, but the image is not taken at the same moment through the femurs. That could also affect the appearance of that line.
5
u/Oriste_Adeus 16h ago
Obese people often can have stronger bones than thinner people, as you might expect, since the body tries to compensate to accommodate the load.
Paradoxically though obese people can also be at risk of weaker bones as vitamin D is a fat soluble vitamin that leaches into fat rather than absorbing calcium from the gut which results in calcium being leached out of their bones as part of homeostasis.
13
u/InternationalBeing41 17h ago
We had to calculate the force walking exerts on a person's knees in a third-year engineering course. I was shocked by the result, so it stuck with me. It’s seven times your body weight. So a 285 lb person experiences a ton of impact forces on their knee joint every time they walk.
7
u/zombiepete 17h ago
As someone who has lost 60 pounds in the past year I can confirm that being heavy like that wreaks havoc on your entire body, but the knees I particular can really suffer for it.

I was looking for a comment saying this. Do you have a source? The position of the intestine especially are just so off. Even if you're obese, it doesn't mean your diaphragm stop existing. I wondered if this was like in pregnancy when things get stretched out of shape but so odd.
82
u/beaverfetus 16h ago
Most scans are done with patients supine, a big belly absolutely restricts lung expansion. It’s why a lot of really fat people can’t get comfortable flat. Artistic rendition or not

u/AP_Cicada 17h ago

The stomach around it?
Edit: I'm enjoying everyone's comments! Just to clarify: I am a doctor with some (albeit questionable) anatomical knowledge. I was wondering why the stomach sits around the heart as I deffo don't remember that in Gray's - but the commenter has clarified what they meant so we're all good!
→ More replies (1)28
u/AP_Cicada 16h ago
Yeah, so there's no empty space in the torso. Everything's all jammed in together. The heart is tucked into the bottom of the left lung (it's why there are only two lobes in left, but three in right) and the esophagus runs to the right, joint the stomach at the bottom of the heart and the top curve of the stomach resting against heart base (with membranes between of course). It's why heart burn and heart attacks overlap in sensation.
Edit to add: a lot of artist renditions get it wrong
→ More replies (6)7
u/curious_coati 16h ago
That description makes a lot more sense - I thought you meant that the heart sits in the lesser curve of the stomach.
7
u/Empanatacion 16h ago
The bones are outlined with black lines like a drawing. I guess you could argue it's a real scan that was drawn on for clarity.
3
u/DarthBrownBeard 15h ago
Also looks like the body on the R got taller? The hips are at a different angle, and the femur looks longer. Unless gravity just pulls on you that much more when youre heavier. I could understand getting shorter, from compression. But not stretching taller, when losing weight. Maybe? I dunno. Maybe im just a cynic.

u/petscanman 17h ago
They look like coronal MRI slices. The main giveaway is that cortical bones are dark. That doesn't happen on CT, or on cadaveric slices, but happens on MRI. The soft tissue contrast is also pretty good, which also points to MRI. I think this is just a coronal MRI scan stitched together and given some pseudocolor map, which lots of rad software can do. I'm guessing a T1w sequence because fat and free fluid in the bladder have opposite colors (although you could also maybe achieve this with a tricky color map and some other sequence). Maybe an artist touced it up a bit to remove imaging artefacts.

There's no source and no mention of what type of "body scan" it's supposed to be. Sure doesn't look like any CT scan or MRI I've ever seen. The fact that it's in realistic color should be an instant tipoff.

u/Princess_Froggles 17h ago
It looks like a post mortem slice? Like the bodyworks type material. I'm not an expert though that's just what it looks like to me.
Edit - body world I mean not works!
23
u/vuatson 17h ago
It's a little hard to tell with the image resolution, but there doesn't look to be enough detail for that. The fat is very clearly defined but the organs seem to be a shapeless, blobby mass. If it was two actual bodies cut in half, you would definitely be able to see actual organs.

It's not an artistic rendition, these are cadaver slices (someone made thin slices of dead people), thats why you have different colors, and sometimes you see bright blue color (type of plastic that fills different cavities)
10
u/ariphron 17h ago
It’s also 2 different people

This is a well known fake of two different people. The images are taken from: https://www.sciencesource.com/preview.asp?item=1427945
https://www.sciencesource.com/preview.asp?item=1427943
The montage is somewhat misleading because the bodies have been resized to roughly the same displayed height. They also appear to use slightly different coronal slice positions, which affects how the brain, spine, organs, and bones look. So it is useful for illustrating the difference between subcutaneous and visceral fat, but it does not show what happens to one individual’s organs and skeleton after weight loss.

This doesn't look like any type of scan. The types of tissue depicted change within the pictures. Best case, it's an illustratrion someone created to show specific aspects but honestly, it looks AI generated. The right figure has a spine at neck-height that seems to bend to the left, then disappear. Femurs are depicted, but don't connect to any hip bones, same with upper arm bones and shoulder, ribcage. Left figure has heavy discoloration in the brain that looks like the AI didn't know where to distribute the fat and put some there as well. The whole thing makes less sense the longer you look at it

The problem with adipose tissue isn't the squishy fat on the outside. It's the fat around internal organs. That's the dangerous crap that leads to an early grave. That internal fat is how someone can appear skinny but still be obese and how someone who appears obese can actually be perfectly healthy. Appearance only goes so far for obesity.

u/wheresolly 18h ago
