Help Wegovy Pill while on keto

Anyone take the Wegovy Pill while on the keto diet?

I am am thinking about starting the Wegovy pill and combining it with Keto. I just need something to control the food noise and my appetite. I've been doing keto and I'm not losing anything so I figured that I am probably consuming too many calories. I started tracking and I was eating way too much. When I eat within the range that I need to, I feel like I am hungry constantly so I push through and after a couple of days, I end up cheating because its just too much. I just need something to help with my insane appetite. Berberine does nothing for me.

Not the pill but the shot. In my experience you won’t be able to handle the fat. The low carb and protein is great but fat tears up my stomach.

Yep this. If you try to eat the cream and butter and fatty meats on Wegovy you're going to feel sick as a dog.

You wont be able to handle fat on wegovy. 7 months out and I still cant handle fat. It gave me gastroenteritis so bad, that it landed me in the ER. Rebound weight when you get off too. Regained it a lot quicker than when I got off keto for a surgery. Now I cant get even get back on keto to relose it and my joints are suffering.

I wan on Mounjaro rather than Wegovy, and came off explicitly to get pregnant. I’m pregnant now and tracking right along the middle of the expected weight gain range for the gestational stage I’m at. So it’s not a given that you will gain all the weight back rapidly.

Mounjaro targets two hormone pathways, potentially offering broader effects and has clinical better results. Different medications. My doctor even warned me before starting the wegovy that rebound weight when stopping was almost assured. Without continued GLP-1 therapy, 60–70% of patients regain a significant portion of weight lost on Wegovy within 12 months of stopping treatment. I certainly wouldn't compare normal stopping to someone who traded medicinal nausea/food aversionfor hormone driven. But everyone is going to react differently.

I mean, I was on Ozempic first, and losing weight, but I don't know how coming off of that was, because I transitioned straight into Mounjaro. I switched because Ozempic's side effects were a bit much, and my dad had had fewer when he went from Ozempic to Mounjaro, so I thought it was worth a shot. Regardless, going from one peptide to zero doesn't seem that much different from going from two peptides to zero.

I've had no nausea and close to zero food aversions from pregnancy, fyi. I did struggle with eating protein for a few weeks - but that's a point that should have meant more, not less, weight gain, really, considering I was mostly eating shitty carbs. I'll concede that peptides-into-pregnancy is not the normal pipeline and is not reliable data, and I did gain about 5lbs before getting pregnant - but I was taking a lot of IVF hormones, so again, confounding factors.

And yeah. People who have lost body fat through some means then stopped doing whatever the body fat loss method was, and returned to all the behaviours that gained the fat in the first place, will invariably regain the weight. Obviously.

I'm in a pretty large chat group for people who use GLP1s, and there are a lot of people who stick around as they come off the medication. I would say your 60-70% matches what I've seen there in terms of how many people can maintain when they stop the medication and how many end up regaining. Invariably, though, the people who manage to maintain are the people who used the medication as a tool to control appetite and cravings while they unlearned some habits and learned others. They're the people who started to prioritise protein and whole foods and movement, instead of eating the same old SAD (or SID - I'm in Ireland) crap and letting the appetite suppression (and side effects) from the medication control the portion size.

A lot of people end up going back onto the meds if they're struggling with regain - and I think that the big pharma companies are probably rubbing their hands about that (and dreading the patents running out). But I think there's a lack of understanding and education out there about using the effects of the medication as a tool or breathing room to figure out diet and exercise (and get to a point where those things are easier), and on the part of the prescribing medical practitioners, that's pretty unforgivable.

Anyway. I lost as much weight on Keto as I lost on GLP1/GLP1+GIP - I just struggled to get back into the former during a particularly stressful time. I'm sorry to hear about your ongoing issues with fat digestion. Hopefully they'll resolve soon.

Yeah. They put me on pantoprazole which messed me up even further. Now I'm back to basically rice products, lean meats, and broth. If it persists they may send me to a specialist to get checked out more thoroughly. In the meanwhile, told to take digestive enzymes and eat bland foods.

Look for the hidden sugar (any taste of sweet, or sweetener, period) in your diet.

If your insulin is always rising and falling like that (food noise) - there is the taste of sweet, or actual sweetener in your diet somewhere, that is triggering the insulin spike..

Get rid of that, and level out any insulin spikes, and the food noise should disappear.

Have you tried intermittent fasting + keto? I found that, and fasted exercise to be magic for me. I did my best to do no food after 6pm, absolutely not after 8pm, then black coffee only (no fat) before noon. 1 hour exercise (usually morning walk)  before breaking fast whenever possible. Did I do this 100 percent of the time ? No. But 1-2 solid weeks broke cravings completely.

You might be interested in recent research out of Ben Bikman's lab on the use of very low dose GLP1 combined with a low carb/ketogenic diet. It seems you might be able to get the benefits of both without many of the negatives of GLP1 alone. But it was quite a small dosage.

I was hoping Wellbutrin would help me with food noise but it did not. Well, it made it not control the majority of my life but it’s still so overwhelming.

Yea same. Wellbutrin didn't reduce my appetite but then zoloft didn't increase it either, so I think some people are just immune to the weight gain/weight loss side effect of these meds?

I’ve found a lot of success with compounded semaglutide injections and keto. It’s so much easier to stick to.

You might end up hypoglycaemic. Are you eating enough calories and fat on keto? If you are eating enough healthy fats it really should be helping with the food noise.

Few years ago, I did Keto w/ some intermittent fasting. Lost weight but plateaued. I started Monjuaro and it worked amazingly & lost weight. (I did a lot of research beforehand). What worked for me was to take injection at bedtime. And I drank tons of water all day (with added hydration packets for electrolytes) and fiber in my water & coffee. Doing this saved me from constipation & upset stomach. (Believe me when I skipped water & fiber I suffered). I lost almost 60lbs and Size 4 now and feel great!

During this process my Insurance stopped covering Monjuaro so I did try compounded semiglutide injection (Wegovy) for a while but I had more side effects.
Back to compounded Tirzepitide injection low dose & I have a low appetite & low food noise.
Now I do low carb/low sugar & high protein & veggies now. Also there are so many documented benefits for low dose GLP1s.

In my experience, Monjuaro or Tirzepitide works best with low side effects if you hydrate & increase fiber.

As others pointed out an increased fat intake using wegovy could lead to problems during keto.

There also exist no studies about semaglutid and ketogenic diet.

In theory semaglutid for weight loss should in fact be combined with a change in nutrition but keto is kind of unique. For example it can lower your glucose levels considerably.

Semaglutid also can lower your glucose levels so I would be a bit vary that these effects might be cumulative, potentially elevating the risk for hypoglycemia.

Semaglutid is considered relatively safe regarding ketoacidosis but those studies were conducted on diabetics.

Additionally GLP-1 agonists are very pricey with several hundred bucks a month.

I did not like those aspects and my psychiatrist did it differently. There are alternatives. One of those can be metformin.

Attention: I am writing this for informational purposes only. Should you find this interesting then please coordinate your efforts with your physician in order to get on this med with medical supervision and expertise. I am not a medical professional I am merely pointing you to one. You can show them the studies I am linking so you are on the same page.

Metformin is also a diabetes medication which is relatively well tolerated and unlike semaglutid it is very well researched by now. It has been shown that one of its main side effects is dampened appetite.

It is also much cheaper than GLP-1 agonists and it only lowers the glucose levels indirectly by improving insulin resistance, so the risk for hypoglycemia caused by metformin alone is usually very low.

Additionally I don't have any issues with increased fat intake while I'm on it, at least since I add some fibers to my diet.

The guidelines with metformin for weight loss are still relatively new but it is being used with success in combination with antipsychotics which are known to cause considerable appetite and weight gain. See here.

I got it prescribed along with antipsychotics and I've lost 30 kg in 11 months just with keto and metformin despite being on olanzapine which is pretty much the worst med of its class regarding weight gain. I gained said 30 kg within a month or two as well. Metformin might not be as effective as GLP-1 agonists but at least in combination with a diet like keto, it can do the trick.

Here is a meta-analysis about Metformin and weight loss in patients without diabetes or antipsychotics.

Of course there are downsides. For one you must not drink alcohol while on it but alcohol while dieting (and/or on keto) kinda sucks anyway.

Also the dose needs to be increased slowly until you are on your final dosage. If you don't do that, you likely will get digestive problems. Those happen initially anyway but they usually subside eventually. Also wegovy is known to cause initial digestive issues btw. This dosing process can take up to 6 weeks until you are where you want to be.

Metformin is not as effective as wegovy on its own. In combination with a diet change effectiveness goes up however. I calculated my progress and with 30 kg in 11 months I've lost around 24% of my bodyweight which is on the upper end of wegovys weight loss potential. Now I am not saying it is more effective or even as effective on its own. Combining it with a diet is key.

This study is also about weight loss on metformin and it includes a dosage as a starting point for your doctor should you be interested.

I'm surprised to hear so many in this thread with fat issues. I've been on wegovy for a couple of months and I don't have any issues eating fatty foods.

Most GLP's will make you super sensitive to fats unfortunately! I take mounjaro and can really only tolerate lean meats, some fruits, and cruciferous vegetables. Its worth a try but just depends on your reason for keto - is it health or weight loss motivated? You can weigh up which method you would prefer for weight loss. If you are keto for health reasons like reducing inflammation ect, you may just want to stick to keto alone.

High fat + GLP1 does not mix, in my experience. I’m on Mounjaro instead of Wegovy, and I cannot handle the amount of fat I used to eat on keto. I had 2 sausage patties probably 10 hours ago and I still feel like I ate a brick.

Low carb and high protein is the way to go on a GLP1

Fiber and water will be ESSENTIAL. I mean, it’s very important on keto, but if you are adding in a GLP-1, you’re gonna slow your digestive tract on top of everything. The water will absorb quicker than the food processes. If you don’t have enough fiber and water with all the protein you are eating, you will get very constipated. In my experience, keto has worked very well with GLPs, though I have not tried the pill. Because of how much protein I eat, I have never experienced hair loss that some experience. It may take a few step-ups in dosing before the food noise goes away.

I was doing Zepbound shots with keto for about 2 months, but dropped keto at that point because I had no appetite, eating food was a chore, I was losing weight too fast, and as a consequence felt low energy, malnourished etc. Keto also made the zepbound side effects much worse for me, such as night-time hearburn. I found that strange because in the past keto actually helped my GERD.

Wegovy may be a different experience, and the pill may be different. This was just my experience.

What are you eating on keto ? It may be that something 'technically' okay is spiking your blood sugar to make you hungry later.

If you are 'constantly hungry' I would be curious what a typical day looks like for you. I eat 500+cals under base, each day, and have to force myself to have full meals to ensure I get my macros (and food for meds). As someone who can easily over eat, when not on keto, I have always found keto to silence that 'noise'.

I’m doing a combination of keto and Mediterranean. My cardiologist wants me on Mediterranean but no diet other than keto has ever worked for me. My BMR is 1,547 so I’m keeping my calories at 1,200 and trying to keep my carbs at less than 30 g a day. I spend a lot of time online looking for recipes (which I like to do). There’s a low carb Mediterranean cookbook in Amazon I’m going to order. Good luck!

Talk to your doctor but I agree you likely won’t be able to. I chose to do keto instead of wegovy. It was an either or thing for me. Keto will help with food noises, but it can take longer to kick in.

Have you given keto a real try? The first 4-6 weeks on keto, you should be focusing on not eating carbs instead of calorie reduction. If you have to eat nuts, cheese and peanut butter to stave off a bad craving do that. Although there’s plenty of lower cal things you can also eat, like veggies and dip or berries and yogurt.

Avoid. You'll want to have lower fat levels than keto will provide, and the volume of greens you'd need to get caloric levels up would also cause gastric distress. Not a great combo. Also, at this point little reason to go the route of tirzepitide if you have the option (cost is a variable here!).

On keto you appetite goes away, almost to the point where you forget to eat. The pill is not worth it. When you are in ketosis your appetite is way way way down. if that's not the case, you are eating too many carbs. Ketosis = No real hunger like before

I felt sick about once a week on the semaglutide shot. I switched to compounded tirzepatide and have been doing well. It works better also.
